Referring a Patient
Simple Psychiatry Clinic welcomes referrals from GPs and other healthcare professionals. Dr Jae Hee Cho accepts patients aged 16 years and over for assessment and management of a broad range of psychiatric conditions.
How to refer
Write a referral letter
Address the referral to Dr Jae Hee Cho, Consultant Psychiatrist, Simple Psychiatry Clinic. A standard GP referral letter is valid for 12 months; a specialist referral is valid for 3 months.
Send it to us
Email your referral to admin@simplepsychiatry.com.au. Please include the patient's full name, date of birth, contact number, and Medicare number.
The patient contacts us
The patient can contact our administration team directly to arrange a suitable appointment time. You will receive a consultation report following the appointment.
What to include in your referral
- Patient's full name, date of birth, and contact details
- Medicare number and concession card details if applicable
- Presenting concerns and reason for referral
- Relevant psychiatric and medical history
- Current medications and any known allergies
- Any previous psychiatric assessments or reports
- Risk factors or safety concerns, if relevant
Medicare item numbers
Patients with a valid GP or specialist referral are eligible for Medicare rebates. The applicable item number will depend on the type and duration of the consultation. Please visit the Medicare Benefits Schedule website for current rebate amounts.
Urgent referrals
If your patient requires urgent psychiatric assessment, please contact us directly by phone. For patients in crisis or requiring emergency care, please direct them to their nearest emergency department or call 000.
