Simple Psychiatry (ABN pending) is committed to protecting your privacy and handling your personal information in accordance with the Privacy Act 1988 (Cth), the Australian Privacy Principles (APPs), and the Health Records and Information Privacy Act 2002 (NSW). Simple Psychiatry is a private psychiatric practice operated by Dr Jae Hee Cho (FRANZCP), located at Suite 17/247 Ryedale Road, Denistone NSW 2114. We provide specialist psychiatric assessment and treatment services to adults aged 16 and over.

We collect personal information necessary to provide psychiatric care, including: your full name, date of birth, contact details (phone, email, address), Medicare number, referral and GP details, health and medical history, presenting concerns and symptoms, current medications, and prior psychiatric or psychological treatment history. We also collect information you provide when submitting an online booking request or contact inquiry through our website.
3. How We Collect Information
We collect information directly from you when you submit an online booking request, complete a contact form, attend an appointment, or communicate with us by phone or email. We may also receive information from your referring GP or other treating health professionals with your consent.
4. How We Use Your Information
Your personal and health information is used to: provide psychiatric assessment and treatment; communicate with you about appointments and your care; process Medicare claims and billing; liaise with your GP and other treating practitioners; comply with legal and regulatory obligations; and improve the quality of our services. We do not use your information for marketing purposes without your explicit consent.
5. Disclosure of Your Information
We may disclose your information to: your referring GP and other treating health professionals involved in your care; Medicare Australia for billing purposes; SIRA (State Insurance Regulatory Authority) where applicable; and other parties where required or permitted by law. We will not disclose your information to overseas recipients unless required by law or with your consent.
6. Storage and Security
Your information is stored securely using industry-standard safeguards. Electronic records are protected by access controls and encryption. We retain health records for a minimum of 7 years from the date of last entry, or until a patient turns 25 years of age (whichever is later), in accordance with NSW Health Records legislation.
7. Access and Correction
You have the right to access and correct your personal information held by us. To request access or correction, please contact us at admin@simplepsychiatry.com.au or call 02 7267 6782. We will respond to your request within 30 days. In some circumstances we may decline access as permitted by law, and we will explain our reasons in writing.
8. Online Booking and Website Data
When you submit an online booking request through our website, the information you provide is stored securely and used solely for the purpose of scheduling and managing your appointment. Our website may use cookies and analytics tools to improve user experience. We do not sell or share website usage data with third parties for commercial purposes.
9. Complaints
